In relazione all'Ordinanza del Presidente della Regione Puglia n. 245 del 2 giugno 2020 che impone a chi si sposti, si trasferisca o faccia ingresso in Puglia, da altre regioni o dall’estero di segnalare lo spostamento, il trasferimento o l’ingresso mediante compilazione del modello di auto-segnalazione disponibile sul sito istituzionale della Regione Puglia, dichiarando il luogo di provenienza ed il comune in cui soggiorneranno e di conservare per un periodo di trenta giorni l’elenco dei luoghi visitati e delle persone incontrate durante il soggiorno, si precisa che:

1. Per i nuclei familiari è sufficiente compilare un unico modulo a nome di un rappresentante maggiorenne del nucleo familiare, indicando nel campo apposito la numerosità del nucleo familiare.
2. Nel modulo di autosegnalazione bisognerà indicare solo il comune ed indirizzo della prima località in cui si sia soggiornato. I successivi spostamenti saranno annotati nell’elenco dei luoghi visitati.
3. Il modulo di autosegnalazione andrà compilato per qualsiasi soggiorno che abbia previsto almeno un pernottamento.
4. Il modulo di autosegnalazione potrà essere stampato in forma cartacea o conservato in formato PDF digitale e conservato per tutta la durata del soggiorno.
5. L’elenco dei luoghi visitati è un documento riservato e personale che dovrà essere esibito esclusivamente alle autorità sanitarie in caso si sia coinvolti in una indagine epidemiologica per il tracciamento dei contatti di un caso COVID-19 confermato. Nell’elenco andranno annotati tutti i luoghi visitati (bar, ristoranti, spiagge, attrazioni culturali, ecc.) e le persone che si sono frequentate e di cui conosciamo l’identità, in maniera appunto, di facilitare l’azione mnemonica in caso di indagine epidemiologica. Allo scadere dei trenta giorni il documento, sia esso cartaceo (una agendina) che digitale (una nota su supporto informatico), può essere distrutto.
6. Le strutture ricettive ricorderanno ai visitatori gli obblighi contenuti nella su menzionata ordinanza attraverso la distribuzione di depliant o l’affisione di poster informativi messi a punto dalla Regione Puglia.
7. L’utilizzo della app IMMUNI sul territorio regionale è fortemente raccomandata, a complemento delle attività di facilitazione del tracciamento incluse nella su menzionata ordinanza.

Clarifications on the provisions about entering Puglia from June, 3rd

In relation to the Order of the President of the Apulia Region n. 245 of June 2nd 2020, which requires those who travel, move to or enter Apulia from other regions or from abroad, to report the movement, transfer or entry by filling in the self-report form available on the institutional website of the Apulia Region, declaring the place of their origin and the municipality in which they will stay and to keep for a period of thirty days the list of places visited and people met during the stay, it is specified that:

1. For families, it is sufficient to complete a single form in the name of an adult representative of the family, specifying the number of the family members in the appropriate field.
2. In the self-report form, only the municipality and address of the first place where you stayed must be indicated. Subsequent movements will be noted in the list of places visited.
3. The self-report form must be completed for any stay that has included at least one overnight stay.
4. The self-report form can be printed in hard copy or stored in digital PDF format and kept for the duration of the stay.
5. The list of places visited is a confidential and personal document that must be presented exclusively to the health authorities if you are involved in an epidemiological investigation to track the contacts of a confirmed Covid-19 case. In the list, all the places visited (bars, restaurants, beaches, cultural attractions, etc.) and the people whose identity is known must be recorded, in order to facilitate the mnemonic action in case of epidemiological investigation. At the end of the thirty days, the document, both in paper (a notebook) and digital (a note on IT support), can be destroyed.
6. The accommodation facilities will remind visitors of the obligations contained in the aforementioned order through the distribution of brochures or informational posters produced by the Apulia Region task force.
7. The use of the IMMUNI app on the regional territory is strongly recommended, in addition to the tracking facilitation activities included in the aforementioned ordinance.
